Many of the House of Assembly's records are available to the public without the need of an official access request. Before filling out the application form, you may choose to contact the House of Assembly Access Coordinator or one of the Statutory Offices to discuss the records you are interested in. Often, you can get the information you need by asking informally.
If you determine that you must make an access request to obtain needed information, you must apply in writing. The Access Application Form is available for download, and is also available in paper form at the House of Assembly and the Statutory Offices. The PDF form can be filled out electronically.
Please send your completed application to the House of Assembly or the Statutory Office you believe holds the records you are seeking. The Access Coordinator will notify you of fees for locating, copying or printing, other formats for reproducing records which cannot be reproduced or printed in the House of Assembly or a Statutory Office and the actual cost of shipping records. The first 15 hours of locating the record(s) are free of charge.
Provide enough information on the application form to allow the House of Assembly or the Statutory Office to identify the records you are seeking. Also, include your name, address and daytime telephone number, as well as a fax number and email address if you have them.
If you are emailing a request to the House of Assembly or a Statutory Office, please ensure the following:
Please note: If you are having trouble filling out/saving/printing the Access Request Form, please follow the steps below which explain how to save the file to your computer and complete it using Adobe Reader.
